چکیده

نوزادان کم‌وزن در هنگام تولد، نوزادانی با وزن کمتر از 2500 گرم در هنگام تولد هستند. این نوزادان بیشتر از سایر نوزادن، در معرض خطرات مرگ‌و‌میر، ناهنجاری‌های مادرزادی، عقب‌ماندگی‌ ذهنی و سایر اختلالات فیزیکی و عصبی هستند. حدود 5/15% از موالید در سطح دنیا، نوزادان با وزن کم در هنگام تولد هستند و کاهش این میزان از موالید به یک‌سوم مقدار فعلی، به‌عنوان یکی از اهداف برنامة یونیسف مطرح شده‌ است. با توجه به موارد ذکر‌شده، پیش‌آگهی تولد این نوزادان، نقش مهمی در پیش‌گیری از وقوع آن دارد. به‌علاوه ، زمینه را برای تصمیم‌گیری‌های بالینی به‌موقع و مؤثر برای حفظ سلامت آنان فراهم می‌آورد. در این پژوهش، از شبکه‌های فازی شواهدی، به‌عنوان مدل پیش‌آگهی تولد نوزادان با وزن کم هنگام تولد، استفاده شد  این شبکه‌ها با استفاده از منطق فازی و تئوری شواهد، قابلیت مدیریت وجوه مختلف عدم قطعیت را دارند. نتایج به‌دست‌آمده از به‌کارگیری شبکه‌های فازی شواهدی و سایر مدل‌های پرکاربرد در این زمینه، مانند درخت تصمیم‌گیری، ماشین بردار پشتیبان و مدل بیز ساده و...، روی پایگاه دادة مربوط به سه بیمارستان شهر تهران، نشان داد که شبکة فازی شواهدی با صحت 84.8% ، عملکرد بهتری از سایر مدل‌ها دارد. در رویکردی دیگر، ترکیب نتایج شبکة فازی شواهدی با مدل بیز ساده، به افزایش صحت پیش‌آگهی به 85.2 % منجر شد. علاوه بر این، توانایی این شبکه‌ها در برخورد با داده‌های گمشده، به‌عنوان یکی از چالش‌های رایج در مجموعه داده‌های پزشکی، بررسی شد. شبکة فازی شواهدی، بهترین عملکرد را در مدیریت عدم قطعیت ناشی از جا‌‌نهی داده‌های گمشده در مقایسه با سایر مدل‌ها، از خود نشان داد و عملکرد آن با افزایش متغیرهای گمشده در هر نمونه، با افت کمتری از سایر روش‌ها روبرو شد.

Babies are born under 2,500 g., defined as low birth weight (LBW) babies. They are exposed to the higher risks of mortality, congenital malformations, mental retardation, and other physical and neurological impairments. 15.5 % of births around the world are LBW. Reduction of the rate of LBW births to one-third is one of the aims of United Nations Children’s Fund program. Prognosis of LBW births can play a critical role in the reduction of these cases. Also, it helps clinicians to make timely and efficient clinical decisions to save these babies' life. In this study, a hybrid framework called fuzzy evidential network with a good ability to manage different aspects of uncertainty is a selected as the LBW prognosis model. The accuracy of prognosis and the performance of the fuzzy evidential network in the management of missing values of the clinical database were investigated and compared with well-known prognosis models of LBW. The results showed that the fuzzy evidential network has higher prognosis accuracy (84.8%) than other prognosis models. On the other hand, the fusion of naïve Bayes and the fuzzy evidential network outputs resulted in higher prognosis accuracy (85.2%). In addition, the fuzzy evidential network performance in the management of uncertainty induced by imputation method, was better than other prognosis models of this study. The performance loss of this framework as the results of the missing data increment, is less than other models.
