چکیده

در این مقاله برای بیماران مبتلا به سرطان پستان که در مرحله III قرار دارند, برای رژیم ترکیبی دوکسوروبیسین و سیکلوفسفامید (AC) در شیمی درمانی کمکی قبل از عمل جراحی، "دز" دارویی بهینه ای طراحی می شود. مهمترین هدف پزشک معالج از اجرای این شیمی درمانی برای بیماران مبتلا به سرطان پستان مرحله ,III رساندن اندازه تومور به اندازه ای مطلوب برای انجام عمل جراحی و برداشتن تومور, بدون آسیب رساندن به پستان بیمار است. رژیم های دارویی بهینه طراحی شده در این مقاله بر اساس شرایط خاص هر بیمار که پزشک معالج برآن واقف است و با در نظر گرفتن مقاومت سلول های سرطانی بیمار نسبت به رژیم دارویی اعمال شده، حاصل شده است. این امر به شکستن سناریوی تجویز مقدار داروی مساوی در مراحل مختلف شیمی درمانی نیز منجر می شود. با توجه به این امر در مقاله حاضر شیمی درمانی کمکی پیش از عمل جراحی به گونه ای فرمولبندی ریاضی می شود که علاوه بر از بین بردن سلول های سرطانی در پایان درمان به بهترین نحو ممکن, سلول های سالم بدن در پایان درمان به بهترین شکل ممکن حفظ شوند یا حالات مختلف قرار گرفته بین این دو مورد از نظر اهمیت را در بر گیرد. در این مقاله با کمینه کردن تابع هزینه ای که شامل دینامیک رشد سلول های سالم و سلول های سرطانی و نیز ضرایب وزنی پیشنهادی پزشک معالج است, رژیم دارویی AC بهینه برای بیماران مبتلا به سرطان پستان مرحله ,III به وسیله کنترل کننده بهینه طراحی و محاسبه می شود.

چکیده [English]

The optimal doses of Doxorubicin and Cyclophosphamide (AC) regimen in pre-operation Neoadjuvant chemotherapy for the patients suffering from stage III breast cancer were investigated. The major benefit of Neoadjuvant chemotherapy is that it can shrink large cancers so that they are small enough to be removed by lumpectomy instead of mastectomy. The optimal regimen designed in this paper was based on the special conditions that every patient had been treated by her/his own physician and the resistance of tumor cells. With respect to these regimen that can achieve non equivalent doses of drug in treatment times for neoadjuvant chemotherapy. The purpose of treating the patients with cancer in neoadjuvant chemotherapy could be either destroying the cancer cells or preserving the normal cell populations in the best way, or different cases between these two situations. In this article, by solving a cost function involved with the dynamics of both cancer cells and normal cells - using the appropriate weighting coefficients suggested by the treating physician- the optimal doses of AC drugs for the patients suffering from breast cancer at stage III were computed by the proposed optimal controller.
