ارزیابی غیرتهاجمی سیستم گلیمفاتیک مغز با استفاده از تصاویر DTI و کاربرد آن در دسته‌بندی بیماران آلزایمر

چکیده
سیستم گلیمفاتیک وظیفۀ پاک‌سازی مواد زائد و رسوب‌های پروتئینی از مغز را برعهده دارد و اختلال در آن، به‌ویژه در فضاهای اطراف عروقی ، در بروز بیماری آلزایمر نقش دارد. هدف این پژوهش، بررسی تغییرات فضاهای اطراف عروقی و شاخص‌های انتشار در افراد سالم، بیماران آلزایمر و افراد با اختلال شناختی خفیف بود. بدین منظور سه مجموعه دادۀ ادنی2، پروژۀ ارتباط انسانی و ادنی4 شامل تصاویر با وزن انتشار، T1w و T2w تحلیل شدند. شاخص‌های انتشار از مجموعۀ ادنی2 استخراج شدند و تحلیل فضای اطراف عروقی در سه مجموعه‌ انجام گرفت. با توجه به تفاوت رزولوشن تصاویر ساختاری در سه مجموعه داده، پیش‌پردازش‌هایی شامل نرمال‌سازی و تصحیح میدان بایاس اعمال شد و در داده‌های ادنی4 و HCP کنتراست تقویت‌شده ایجاد گردید. پس از به‌کارگیری فیلتر فرنجی برای شناسایی خودکار فضاهای اطراف عروقی در مادۀ سفید، شاخص‌های کمی محاسبه و با آزمون‌های آماری و مدل‌های پیش‌بینی ارزیابی شدند. در داده‌های ادنی2، بیماران آلزایمر شاخص ALPS پایین‌تری نسبت به افراد سالم نشان دادند (16/1 در برابر 31/1) و میانگین تعداد PVS نیز به‌طور معنی‌داری کمتر بود (60/84 در برابر 20/112). در HCP، کنتراست تقویت‌شده تشخیص PVS را بهبود داد و این موضوع بر اساس ارزیابی چشمی متخصص تأیید شد؛ همبستگی جزئی میان شمار خودکار و چشمی برای EPC و T2w به‌ترتیب 75/0 و 44/0 بود. در داده‌ی ادنی4، اگرچه تفاوت‌ها معنی‌دار نبودند، شاخص‌های PVS روند کاهشی را در بیماران آلزایمر نشان دادند. همچنین شاخص‌های انتشار و معیارهای بالینی در شناسایی تغییرات مرتبط با آلزایمر نقش ارزشمندی داشتند. با توجه به محدودیت حجم نمونه و رزولوشن تصاویر، استفاده از داده‌های بزرگ‌تر در مطالعات آینده توصیه می‌شود.

Diffusion weighted images The glymphatic system is responsible for clearing waste products and protein deposits from the brain, and its dysfunction—particularly within perivascular spaces (PVS)—has been implicated in the development of Alzheimer’s disease. This study aimed to investigate alterations in PVS characteristics and diffusion metrics in healthy individuals, patients with Alzheimer’s disease, and those with mild cognitive impairment.

Three datasets were analyzed: ADNI2, the Human Connectome Project (HCP), and ADNI4, including diffusion-weighted, T1-weighted, and T2-weighted images. Diffusion metrics were derived from ADNI2, while PVS mapping was performed across datasets. Because of differences in structural image resolution, preprocessing included normalization and bias field correction. Enhanced contrast was generated for HCP and ADNI4 images. Perivascular spaces within white matter were automatically segmented using the Frangi filter, and quantitative metrics were evaluated through statistical analyses and predictive modeling.

In ADNI2, patients with Alzheimer’s disease demonstrated a lower ALPS index than healthy controls (1.16 vs. 1.31). The mean PVS count was also significantly reduced in the Alzheimer’s group (84.60 vs. 112.20). In HCP, enhanced contrast improved PVS detectability, as supported by expert visual assessment. Partial correlations between automated and visual counts reached 0.75 for enhanced contrast and 0.44 for T2-weighted images. In ADNI4, although group differences did not reach statistical significance, PVS measures—including count, volume, and volume fraction—showed a trend toward lower values in Alzheimer’s patients. Enhanced contrast further facilitated PVS visualization and quantification.

Overall, diffusion-derived indices together with clinical variables contributed to detecting Alzheimer’s-related alterations. Given the limited sample size and image resolution, further investigations using larger and more homogeneous datasets are warranted.
